A pharmacist is a health care professional who has taken specialized training in drug uses and roles, potential drug interactions and side effects. In treating patients a pharmacist may also educate them on over-the-counter vs. prescription options, avoiding alcohol while taking certain medications and avoiding drug interactions. The Role of a Clinical Pharmacist in Cardiac Surgery
EH: So the pharmacist will review what medications – both prescription and non-prescription – that need to be changed, adjusted or added before surgery, and exactly when those changes should be implemented. We will provide you with a written copy to take home with you about these instructions. So the pharmacist will take into account all your medications and ensure that we’re screening any new or stopped medications for interaction to keep you safe.
YS: As a part of the cardiovascular surgery team, your pharmacist will need to obtain an accurate, up-to-date list of your prescription and non-prescription medications, as well as any allergies you might have experienced. You will need to bring all of your prescription as well as non-prescription medications in their original container for this visit. This will include any vitamins or other over-the-counter products.
Although we prefer the actual medication bottles to help you identify your medication, an up-to-date list from your community pharmacist is also acceptable. As well, please bring the list of any allergies to any medication, and the last reaction that occurred from the exposure.
You might find that there’s a lot of information provided during this visit. Do not be afraid to ask a friend or a family member to accompany you to this visit. All cardiovascular surgery patients receive education on their medication from a cardiovascular surgery pharmacist when they’re discharged home. This ensures that when you do come home you understand what changes were made to your medications, and what type of follow-up you need.
EH: So as part of the cardiovascular surgery team, your pharmacist will be able to address any medication concerns throughout your admission. Please remember that this visit is also an opportunity to have your questions addressed.
